I hereby assume all responsibility for injuries I may receive or articles lost while participating in this event and hereby release the Auditorium Theatre of Roosevelt University, Hiplet Ballerinas of the Chicago Multi-Cultural Dance Center, and their respective staff, boards, and guest artists from any liability for any injury I may sustain.

If participant(s) is under 18, I hereby give my child permission to participate in The Auditorium's Community Workshop Class.
